Yes, I'm pretty sure he'll need one.
I believe that after June 1st everyone needs them.
You will also need a Notarized "Parental Travel Consent Form" signed by his mother authorizing permission to remove him from the U.S.
It doesn't matter if your still married, divorced, or if she's deceased (proof of death is required).... a notarized form is required if she is not traveling with you.
There is no "official form" however there are a list of suggested guidelines and several downloadable sample forms that you can have her fill out and get notarized.
